After completing the online registration please bring the following documents to the registration office.

  • Birth Certificate
  • Parent Photo ID
  • Social Security
  • Address Verification
      • Water or Power Bill in the custodial guardian's name. **If the Water or Power Bill is not in the parent/guardian's name, a Notarized Residency Affidavit can be used accompanied with a water or power bill in the signers name. > Download Affidavit Here.
  • Alabama Shot Record
  • Withdrawal from the previous school
  • Judge Signed Custody paperwork (if applicable)

  • Récord de vacunas de Alabama
  • Documentos de Retiro de la escuela anterior
  • Documentos de custodia firmados por un juez (si corresponde)
